Pulaski Adult Day Service & Fall Prevention Center

Pulaski Adult Day Service & Fall Prevention Center serves people 18 and older who are dealing with being alone during the day, a range of physical problems, or early to mid-stage dementia, and would like to join a program which provides interesting conversation, goofing off, laughing, good food, care, and companionship in a safe and cheerful environment. Our mission is to keep adults living in the community with their families rather than being institutionalized. We provide care Monday- Friday: 9am - 5pm.

We monitor nutrition and hydration providing breakfast, lunch and an afternoon snack. Sitting around the table and enjoying meals together builds a sense of community. Exercise within each person's ability is a part of each day. Making exercise fun is a great way to stimulate activity. 

Our Registered Nurse and Certified Nursing Assistants provide compassionate care for adults 18 and older with needs such as assistance with eating, toileting, ambulating, dressing/bathing and medication administration. We assess and monitor health conditions and alert family members to changes in health, which helps prevent hospitalization.

We love being outside planting, walking and just enjoying the Therapy Garden (the NRV Master Gardeners come to teach us about plants and provide Garden Club), playing games building on language and math skills, cooking together, music and singing. We have a Certified Music Therapist who taps into memories through music and has a gift for reaching those who have difficulty expressing themselves. We have had people to sit down and start playing the piano after years of not playing. Learning new skills and even playing an instrument for the first time are possible when we all play together. Individual engagement and support enables socialization which improves quality of life.

We provide several research based programs including A Matter of Balance, Chronic Disease Self Management Program, and Bingocize. 

We serve a very diverse group with varying abilities who are accepting of new members and make them feel at home. We learn from each other every day. Many of our people have raised a family through hard times and served our country and helped build this community. So, they deserve our best and it is an honor to know and serve them and their families.
